Sports Physicals For 2023-2024 School Year 

August 14th - 8am -10:30am at the Middle School

August 21st - 8am -10:30am at the High School

Please bring a completed Health Interval Form to be seen by the doctor.

**All athletes MUST CHOOSE A SPORT and be REGISTERED and CLEARED by the school nurse on Final Forms to be eligible to participate on a sports team**

Final Forms will open  for Fall on or about  - July 22nd for Varsity & JV and August 8th for Middle School
